This document shall serve as the annual authorization for the financial statement of the

to be included in the NAPNAP Inc. group tax return for the year July 1 to June 30.

If filing by hard copy:
ALL OFFICERS MUST SIGN. ONLY ORIGINAL SIGNATURES WILL BE ACCEPTED. NO FAXED COPIES! IF APPROVED BY YOUR EXECUTIVE BOARD AS DOCUMENTED IN MEETING MINUTES, THIS FORM MAY BE SIGNED BY ONE OFFICER AS DESIGNATED.

By entering your name and submitting the form, you indicate that you have read and understand the above; attest that you are authorized to speak for the chapter; and hereby request that NAPNAP include your chapter in its Group Income Tax filing for the current fiscal year.
