Use this form to document funds received from the National Office for chapter membership, number of chapter members and any membership amount collected by the Chapter by quarter.

The deadline date for each quarter are listed below. Please reference the “Chapter Paid Dues” file to identify the members that have paid the National Office in the appropriate months listed in the Dues Date Range column.

Note: the months listed under Dues Date Range column are for when the National Office collected the membership dues, not the months when your chapter receives the “Chapter Paid Dues” file.
